First of all this is the tutorial I used HERE and I more all less followed it with a tip for sticking the corners I used some bulldog clips to hold them in place while they stuck.
I've use one of Mo Manning's images Urchin with bird and the papers are a download which I've just spent the last ten minutes trying to find the name but I've given up.
I've used gold card with the magnolia swirl and the Martha Stewart holly punch.

I've used the top note die from stampin up for somewhere to write your greeting and again more holly to embellish.
The book inside do think this is clever.
I found this poem on online, I'm hoping you can read it better when enlarged.
